Most battery Isolators are effectively giand diodes. The current is fed from the alternator through the isolator and to each of the batteries. Current can not flow between the batteries however (this is the isolation part). You connect the stereo to one battery and run the car off the other.
The drawback is that there is about a 0.7V drop across the isolator, so your alternator isn't as effective.
