Yes they are really speakers. The usually get mounted under the dash, but I have seen some crazy people put them IN the dash as well. Some think they are worth the cash, some think they are junk.
Horns will give you a broader range of frequencies than a tweeter will. They typically give you a great soundstage, but there is alot of tweeking and equaization involved to make them sound just as great.

I heard Joe Blacks car this WE and he has horns in there, under the dash. The horns did what horns have always been best at ...sounding real and being dynamic. He had a lot of EQ and ran the bulk of the freq spectrum through them... going from moderatly quiet to full on loud there was no tonal shift it just got loud. Not all horns, or cars, sound the same so EQ will vary.
