I had the same thing happen to my spl12 sub. It's just the dustcap that comes unglued and takes the top layer of thin paper with it. If the cap was glued to the actual cone and not the paper it would probably hold better.
In the later models of the subs mach5 was drilling holes in the dustcaps to relieve the pressure.

If it's tearing the paper it is lifting. Just give it some time and it will come loose. Took about 2 months with mine like that to realize what was happening. I thought I had folded the cone, but it was still burping fine.
^^how did you guys handle warranty claims with those versions? That's the important part and determines how the company should be treated with regards to new products. Did you require the purchaser to pay for shipping back to you etc..
I would never hesitate to buy from Mach 5 Audio ever again.
Also, the issue causing it was no vented pole piece. Looks like we have one on the NEO
