Just take a grinder and take off the paint about 2"-3" from where the rust is, then grind off the rust, or media blast it all off. once the rust is all off then you should use a metal conditioner to neutralize the metal. all your filler.....you should NEVER apply filler more than 1/8" thick or you can easily run into problems. sand it smooth prime it and paints it easy as that.
